IT'S EASY! Simply take a video of your golf swing from face on and down the line, and send them to one of our Award Winning Coaches, through the Onform Video Analysis App. It's as easy as sending a text message!

Once you've submitted your swing videos, you'll receive a detailed swing analysis using Onform's analytical tools with a voice-overlay explaining your areas of improvement and how it's affecting your performance. Then, you'll receive a personalized video demonstrating exactly what you need to work on with drills and a practice plan!

Once you purchase your membership, we will send you an email to connect with your instructor on the Onform App!

Membership Policies:

*Memberships are billed monthly on the 1st (unless paid annually) and automatically charged to the card on file.

*Unused lessons may be carried over to next month; units expire at the end of each month.

*Memberships can be upgraded at any time or downgraded at any time. If downgraded you must wait 3 months to upgrade.

*To cancel instruction membership, a written notice (email or letter) must be submitted by the 21st of the month prior.

*If membership is canceled, all lessons and credits expire with membership cancellation.

*If an annual membership is canceled early, the annual discount is forfeited, and any refund is prorated at the standard monthly rate.

*If a class is canceled by Level Up, efforts will be made to reschedule; however, no make-ups are provided for student absences.

*All lessons and services require a 24-hour cancellation notice in advance. Cancellations within 24 hour are not eligible for a refund.
